PCOs (Polycystic Ovarian Syndrome)
Polycystic Ovarian Syndrome is a complex condition of Ovarian Dysfunction affect between 5-20% of women of Reproductive Age
Why does it Happen is still not known but Is the most common Endocrine issue being faced by women , what happens Actually is The Ovaries (female organs that lay eggs) are unable to release the ovum because of multiple cysts inside causing hormonal push and multiple problems
This Hormonal push has also been known to cause increased risk of Endometrial Cancer but yet to be proved
What are the Features of PCOs?
It presents with the Features of
• Subfertility/Infertility
• Mentural disturbances like Oligomenorrhea(Decreased amount of menstrual Blood) or Amenorrhea (Absence of Menses)
• Hirsutism ( Increased Facial Hair)
• Acne ( Due to Increased Male Hormones)
• Obesity
• Acanthosis Nigerians (Blackish Skin discoloration)
• Thinning and Hair loss
• Anxiety
• Depression
• Gestational Diabetes
How Would I Know Weather I have PCOs or Not?
• It is diagnosed initially by Getting a pelvic Ultrasound which will show multiple cysts on.the ovaries hence called Poly-Cystic Ovarian Syndrome
• It is also diagnosed by the level of hormones one contains as LH:FSH (Hormones) is raised which is Considered as "Classic" for PCOs but isn't Diagnostic
• Prolactin levels may be raised may be normal
• Testosterone (Male Hormone) may also be raised
• Females with PCOs also have impaired Glucose Tolerance Test
• Both the hyperinsulinemia(increased insulin levels) and LH levels are seen in PCOs
Can it be managed?
Yes But its really complicated because of its unknown Reason!
The most important Aspect of Its treatment and To gain some amount of Fertility is to "Reduce Weight" this first step is also known to reduce the Occurrence of Hirsutism(Increased Hairs) and Acne
Yoga and Exercise has been known to reduce Weight and also improve the mood disturbances that occur with PCOs
Reducing carbonated Drinks, And alcohol has also been known to reduce the symptoms of PCOS
About 50% of women respond just by doing the above steps , rest of them need medications to help them
If a women is not interested in Fertility then Combined Oral Contraceptive Pills are the ones to go, as they help in regulating the cycles and induce a monthly bleed
But for a women needing Fertility, they should be closely supervised by specialists as debates are on weather drugs like Metformin (Used for Diabetes and Weight loss) or Clomifene or both should be used to induce ovulation
A research was conducted in NEJM 2007 Suggested that the Clomifene was the most effective Treatment for the Women having PCO and Need fertility but this yet has to be approved completely, and 2008 the RCOG also informed that the Metformin is not the first line drug! Either it is used with Clomifene or only used to reduce weight in women with PCOs.
